Boy, am I ashamed of my memory.  The manual I was referring to is named
           Guidelines for APL Systems
                    by
            J. Ever and C. Fair
It has a stamped number of: DPD 22 IBM 037301
The address on the title page is IBM, 1000 Westchester Ave., White Plains,
NY 10604.  It is dated March 1, 1976.
 
I guess there is a snowball's chance in hell of getting a copy for most folks,
but if you can locate one, take a look.  For me, it *was* an APL Manual of
Style, so, I guess that is why I thought that was its title.  Unlike other
collections, it is *not* a collection :-).  It gives progressive didactic
examples of how to avoid branching, how to encode data, efficient execution,
etc.  It was influential in causing me to be able to make the leap to
parallel, rather than sequential, problem solving.

Another manual I came across, also probably unobtainable, but I will mention
it anyway, is IBM Technical Report, TR 00.2409, Useful APL Defined Functions
by Kent Haralson, Feb. 8 1973.
